Provide patient care following physician and hospice procedures/requirements, practicing within the scope of license, certification or registration, where applicable, and as required by law and regulation. Essential Duties and Responsibilities:
Provide direct patient care, as attending or consulting physician, to hospice patients in Hospice House facility, nursing homes, assisted living facilities, and/or patient homes/residences
Provide medical aspects of care in collaboration with the interdisciplinary team of attending physicians, nurses, social workers, certified nursing assistants, and chaplains.
Actively participate in weekly Interdisciplinary Team meetings for development and evaluation of the patient’s plan of care.
Participate in the coordination of the patient’s hospice care in accordance with professional standards of practice.
Participate in the development and evaluation of the plan of care.
Prescribe necessary assessment and treatment for pain and symptom management.
Practice safety and infection control methods.
Help facilitate creation and tracking of patient data and records.
Contribute to patient and family counseling and education.
Perform duties in compliance with Medicare Condition of Participation.
Maintain relationships with community physicians.
Act as the designee of the Hospice Medical Director as required based on the needs of the agency.
Take clinical call for hospice patients.
Works to achieve job performance goals, pursues progress toward individual / personal development goals, and demonstrates Partners In Care’s values and competencies.
Perform other duties as requested or assigned.

Doctor of Medicine degree or Doctor of Osteopathic Medicine degree from an accredited institution
1-3 years of applicable experience in the treatment of end of life care
Demonstrated understanding of the Centers for Medicaid and Medicare Services requirements
Experience effectively collaborating with a diverse group
Licensure / Certification:
Physician is and shall maintain the valid and unrestricted medical license to practice medicine in the State of Oregon. Board eligible or Board Certified in Hospice and Palliative Medicine, preferred.
Professional Standards / Compliance with Law: Physician shall at all times comply with the ethics and professional standards of the medical profession; applicable federal, state and local laws, rules and regulations; Partners In Care’s policies and procedures; and the rules and regulations that are applicable to Partners In Care, including accreditation and certification requirements.
Drug Enforcement Administration: Physician has and shall at all times during the course of employment maintain registration with the Drug Enforcement Administration (“DEA”) and any applicable state agency without limitation to prescribe drugs under such registration and shall provide current proof of such registration upon Partners In Care’s request.
Qualify for Partners In Care’s Professional Liability Insurance Coverage: Physician shall at all times qualify for insurance coverage under Partners In Care’s professional liability policy.
Knowledge of Federal, State, and Local regulations and of Medicare CoPs.
Requires excellent understanding of pharmacology, interactions, use of controlled substances.
Knowledge of HIPAA privacy rules and regulations
Knowledge of electronic records systems
Demonstrate excellent attention to detail and accuracy
Proficient in MS Office: Outlook, Word, Excel, etc.

Environmental / Working Conditions:
May be exposed to extremes of heat and cold in all weather conditions. May drive in various weather conditions on roads in varying degrees of repair.
May be exposed to infections and contagious diseases. Contact with patients under wide variety of circumstances.
May be exposed to/occasionally exposed to patient elements. Subject to varying and unpredictable situations.
Handles emergency or crisis situations.
Travel required throughout Partners In Care’s central Oregon area of service coverage including LaPine, Crescent, Christmas Valley, Prineville, Madras, Sisters.
OSHA exposure Category I: Position includes tasks that involve exposure to blood, body fluids, tissues. Required Personal Protective Equipment as conditions warrant including: Gloves, gown, mask, mask with shield, goggles, shoe covers, bouffant cap.
